Candidate Instructions

Chris Brown is a 33 year old married graphic designer. S/he has come to see you today at your GP surgery at the request of his/her spouse who feels Chris’s behaviour has been out of character and generally overactive for the past 2 months. Chris is waiting outside and she has agreed to see you to discuss the problem.

 

Please take a history focusing on the past few weeks.

 

This is a 5-minute station.

 

 

Simulated Patient Script

You are Chris Brown. You are 33, married, no children and a graphic designer, quite successful at your job. For the past 3 weeks you have been arguing with your spouse because you are full of energy and “inspired”. You accept you are irritable at times but you do not think it is a particular problem. You are designing a website for a major company and have a deadline. It is a bit stressful.

 

You have spent £3000 in the past week on clothing because it makes you feel good.

 

You sleep 3 hours maximum per night and feel refreshed. You have no formal psychiatric history but used to get depressed at university.

 

You did drugs occasionally in your 20s and drink alcohol socially.

 

You are over-talkative in the conversation, emotional, dramatic.

    • Introduces self to patient

    • Communication skills

  2. Elicits Symptoms of Mania/Hypomania

    • Over-activity

    • Overspending

    • Reduced need to sleep

    • Increased energy

    • Elated mood

    • Effect on life both personal and at work

  3. Other Symptoms

    • Paranoia

    • Hallucinations

  4. History Prior to Current Episode

    • General functioning (social history)

    • Past history (previous similar episodes or depressive episodes)

  5. Substance Misuse

    • Alcohol and drugs
